Because I live with some health issues that take me to various healthcare providers I fill out and sign HIPPA forms often. I have at my Dr., my Dentist, for physical therapy, when I see my massage therapist, when I have had any surgeries and also when I have done acupuncture.
If you go somewhere that treats you for any health related issues, they must have a form for you to sign.
